How to Apply for Community Grants
The SC Mountains to Midlands Affiliate of Susan G. Komen® is now accepting applications for innovative projects in the areas of breast health services otherwise not available to the medically underserved populations of Abbeville, Aiken, Anderson, Cherokee, Chester, Chesterfield, Edgefield, Fairfield, Greenville, Greenwood, Kershaw, Lancaster, Laurens, Lexington, McCormick, Newberry, Oconee, Pickens, Richland, Saluda, Spartanburg and Union counties. Grants are available for one (1) year, April 1, 2016 through March 31, 2017.
Applications are accepted for programs not exceeding $65,000 for Screening and Treatment grants or $30,000 for Outreach grants. Personnel expenses are considered Outreach expenses. For projects incorporating both outreach and screening/treatment, the maximum funding is $65,000 with the outreach portion not exceeding $30,000 of the $65,000.

REQUEST FOR APPLICATIONS
The Request for Application (RFA) is built around the SC Mountain’s to Midlands Affiliate’s Statement of Need which conveys the guidelines for the types of programs Komen and the Affiliate are willing to fund. The Statement of Need and RFA are both based on findings from the Community Profile conducted every 4 years. The Statement of Need is included as part of the RFA. The RFA also sets the rules for applying for Komen Affiliate grants, including deliverables under the grant contract, evaluation criteria, grants structure, budget and other qualifying guidelines.
COMPLIANCE AND REVIEW
Applications received on, or before NOON on Wednesday, December 3, 2015 will be reviewed for compliance. Applications that do not meet eligibility, submission, formatting or responsiveness requirements will be administratively withdrawn and will not undergo further review. Fully completed applications that are compliant with this RFA will be submitted for grant review by an independent panel established through the local grants committee. Each qualified application is then reviewed by a panel of at least three reviewers with appropriate expertise in breast cancer care, grantmaking, nonprofit administration, patient advocacy, program management, public health, and/or program finance. Reviewers assess the strengths and weaknesses of each application based on the defined review criteria described below. Final funding decisions are approved by the Susan G. Komen SC Mountains to Midlands Affiliate Board of Directors. The grant process is competitive, and funding is never guaranteed, whether or not an organization has received a grant in the past. Funding is never guaranteed for any reason. Each application will be reviewed based on the merits of the program balanced with the priorities of the Affiliate using the following selection criteria: Impact, Feasibility, Capacity, Collaboration, Sustainability, and Breast Health Continuum of Care.
